First, don't ask me how it got it's name, I have no friggin' idea.
Ingredients:
Chocolate syrup. Fox's U-Bet is traditional, it might be hard to find, but Hersheys works.
Whole milk, 2% is OK, but whole milk is traditional.
Seltzer. NOT CLUB SODA!!!
Refrigerate everything including the glass.
In a 16oz. glass, put approx. 1/2" of syrup, then about 3/4" of milk. Stir the two together. Add seltzer to the top and stir gently to mix. If the seltzer is good, you'll have a nice foamy head.
That's it. Drink it.
You can vary the amount of syrup and milk to your taste. The glass I use tapers outward bottom to top. I've used cream soda instead of seltzer, but I prefer the traditional taste. I suppose you could substitute pretty much any soda, but I don't know how it would taste.
When I was a youngster I worked in a soda fountain/luncheonette and I drank my share of these.
